Contradictory and flexible ego functions are required: will the patient be able to regress within a session and recover at the end? Can he renounce some reality testing yet still understand what we are communicating? The patient needs honesty of purpose and integrity; the opposite can sometimes be seen in an assessment.

A patient telephoned sounding convinced that he wanted psychotherapeutic help. Even from the telephone conversation, the therapist was aware of feeling deeply uneasy. She sensed an underlying aggression in his demand for an assessment. Working in private practice, not within a hospital or clinic team, it was important that she would not feel threatened, so she ensured that she would not be alone in the house during the consultation.
